What Is Lower Blepharoplasty?

Lower Blepharoplasty, also called Eyebag Surgery, treats puffy under-eyes and visible fat bulges. The operation removes or repositions selected fat to create a smoother, less tired appearance while preserving the patient’s natural eye shape.

Who May Be a Candidate?

This procedure may be considered for patients with noticeable eyebags, under-eye puffiness, or visible fat pads beneath the eyes. It is most suitable when bulging fat—not under-eye hollowing—is the main concern.

Not every concern that looks like an eyebag should be treated with fat removal. Dark shadows, deep tear troughs, sunken under-eyes, weak cheek support, or a negative-vector eye position may require a different plan. In selected patients, under-eye fat graft transfer may be a better option. Its published rate is ₱60,000.

Will there be a visible scar?

The incision may be placed just beneath the lash line or inside the lower eyelid, depending on the surgical plan. Scars usually become less noticeable with time, but healing varies and no scar can be guaranteed to become invisible.

Will Eyebag Surgery fix dark circles?

Only when the darkness mainly comes from bulging fat and the shadow it creates. Pigmentation, thin skin, visible blood vessels, or under-eye hollowing may need a different treatment.

Can it be combined with Upper Blepharoplasty?

Yes, when appropriate. Upper- and lower-eyelid surgery may be planned in one session after Dr. Bien examines both areas and discusses the added recovery and risks.

When can I wear concealer or eye makeup?

The original guidance is usually around the second to third week, depending on wound healing and stitch removal. Follow the specific advice given during your postoperative visits.

How soon will I see the final result?

Swelling often improves during the first one to two weeks, but the area continues to settle and refine over several months. The original page gives a full-healing guide of about four to six months.

When might fat grafting be a better option?

Fat grafting may be more appropriate when the main concern is a sunken tear trough, under-eye hollowing, or a deep lid-cheek junction rather than a true fat bulge. An in-person assessment is needed to distinguish these concerns.
